Stretching  the anterior thigh (rectus):




a) Patient in a supine position, one leg pulled back to the chest, the other hanging over the edge of the couch and held between the thighs of the therapist. The therapist  grasps the anterior thigh muscles with both hands and applies u-shaped pre-tensioning  by turning the hands. To stretch, the flexing of the knee is intensified.



b) With the patient in a right-sided lateral position, s-shaped pre-tensioning of the anterior thigh is applied by using a pinching grip. To stretch, the hip joint is first straightened.

c) This is followed by the flexing of the knee joint to stretch the anterior thigh.



Stretching the posterior thigh  (hamstrings, ischiocrural muscles):






Stretching the anterior lower leg (tibialis, extensor digitorum)



a) With the patient in a prone position, the therapist places the patient's thighs on his/her own. S-shaped pre-tensioning is created using a pinching grip on the inner or outer side, according to the location of the pain.



a) With the patient in a supine position, the tibial muscles are pushed away from the tibia with thumb pressure. This creates a u -shaped pre-tensioning. The forefoot is grasped with the other hand and straightened to stretch the ankle joint. 


b) To stretch, the knee joint is straightened.



c) The same procedure is used for splayfoot complaints. To stretch, the small toes are grasped and with a straightened ankle joint the toes are flexed and forefoot turned outwards (supinated )


b) In the same manner, the first phalange and big toe are grasped and straightened with the forefoot turned  inwards (pronated ) to stretch the ankle joint.

Stretching the posterior lower leg (triceps, peronaeus)





a) With the patient in a prone position, the posterior lower muscles leg are grasped with both hands, either on the inside or outside, and u-shaped pre-tensioning applied.



b)To stretch, the therapist presses his/her thigh against the patients foot and flexes the ankle joint.


c) For the outer side, the muscles behind the fibula are palpated and pushed away to apply u-shaped pre-tensioning. To stretch, the therapist presses his/her thigh against the patients foot and flexes the ankle joint.


d) For the deeper parts, with the patient in a prone position with a flexed knee joint, s-shaped pre-tensioning is applied using a pinching grip. To stretch, the ankle joint is flexed.



Stretching the foot muscles (Planta pedis)





a) In a prone position, with a flexed knee joint and straightened ankle, u-shaped pre-tensioning is applied by thumb pressure from inside or outside.




b) To stretch, the knee joint is flexed.
