1) The main goal is to treat the hardening of the arm and neck muscles by stretching exercises and by loosening the blockages with mobilization exercises.

|
2) Correction of a pattern of movement is achieved through coordination exercises in which one learns to lift the arms without raising the shoulder blade and to ensure that the arm rotates when lifted
|
 |
Strengthen and move:
a) prone position, forehead held about 1 cm from the ground. Press shoulder blades down, thumbs touching the thighs, arms approximately 1 cm above the ground. Thumbs inside when arms pointing downwards. (Detailed image of the activated lower shoulder girdle fixators)

b) prone position, shoulder blades pressed downwards, hold the arms just above the ground and move them upwards at the same time rotating the thumbs upwards. Thumbs fully up when arms reach shoulder height.

c) prone position, shoulder blades pressed downwards, continue to move the arms upwards whilst just off the ground, at the same time rotating the thumbs backwards. Thumb behind when the arms are above the head.

3.) One can treat muscle tension by local application of a warming cream, whereby we recommend a muscle warming cream made from natural products. This contains the anti-inflammatory devil's claw, the relaxant rosemary oil and a pain-relieving capsicum extract. We use an anti-inflammatory gel for painful tendons in the elbow and possibly in the wrist area, because of its cooling effect and because it contains devil's claw and arnica. As a food supplement for the regeneration of articular cartilage and tendons, we recommend cartilage building capsules prepared from natural products. In addition, if needed, pain medication with anti-rheumatics and, temporarily, bandages.

4) We recommend manual stretching of tense muscles and manual mobilization of the blocked motion segments of the cervical spine, if possible by a therapist. This mobilization process can also be augmented by a neck roll or a neck pillow with mobilizing properties (mobilization pillow).

5) Alongside pain medication / pain therapy with anti-rheumatics, for well-stretched muscles, cortisone injections can be given once or twice in the area of the painful tendons to relieve residual symptoms. This is never the first-line therapy however and only makes sense if the patient has corrected the muscle shortening by stretching exercises.

|
6) In rare cases, when conservative therapy fails, surgery is an option for tennis elbow with a low complication rate. Before doing this however, laboratory tests and imaging procedures should be used to exclude other possible causes of the painful dysfunction.

Summary:
To summarize, the cause of tennis elbow / golf elbow is a hardening of the muscles of the forearm, with painful tendon ends in the elbow and sometimes also the wrists. Treatment is primarily a domain of exercises, as well as local treatment with warm, muscle relaxing creams on the forearm and cooling, anti-inflammatory gels and possibly ice on the painful tendon ends of the elbow joint. Dietary supplementation with joint capsules makes sense and anti-rheumatic medication is used principally during the acute phase. Cortisone injections are a further option but only after conservative treatment.
An operation is only necessary for our patients in rare cases.
